Objective: The underlying neurophysiologic mechanism responsible for secondary paroxysmal kinesigenic dyskinesia (PKD) is still unclear. Here, we study the pathogenesis of PKD in two patients with a demyelinating lesion in the spinal cord. Methods: Electromyogram recordings from affected arms of two patients with spinal cord lesions presenting PKD were compared with our laboratory standards. The cutaneous silent period (CuSP), mixed nerve silent period (MnSP) and coincidence period (CiP), defined as the common period between the CuSP and MnSP, were recorded. Results: A large decrease in the MnSP and disappearance of the CiP were observed in our patients, which was secondary to simultaneous extinction of the third portion of the MnSP, while the CuSP was normal. The MnSP and CiP were normal after recovery. Conclusions: Our results demonstrate that the third portion of the MnSP and the CuSP do not correspond to the same physiologic process. These findings suggest that PKD patients have abnormal spinal interneuron integration.

Age-related changes in the neuromuscular system can result in differences in fatigability between young and older adults. Previous research has shown that single joint isometric fatiguing exercise of small muscle results in an age-related compensatory decrease in GABAB mediated inhibition. However, this has yet to be established in a larger muscle group. In 15 young (22 ± 4 years) and 15 older (65 ± 5 years) adults, long interval cortical inhibition (LICI; 100 ms ISI) and corticospinal silent period (SP) were measured in the biceps brachii during a 5% EMG contraction using transcranial magnetic stimulation (TMS) before, during and after a submaximal contraction (30% MVC force) held intermittently to task failure. Both age groups developed similar magnitude of fatigue (~24% decline in MVC; P = 0.001) and ~28% decline in LICI (P = 0.001) post fatiguing exercise. No change in SP duration was observed during and immediately following fatigue (P = 0.909) but ~ 6% decrease was seen at recovery in both age groups (P<0.001)." Contrary to previous work in a small muscle, these findings suggest no age-related differences in GABAB mediated inhibition following single joint isometric fatiguing exercise of the elbow flexors, indicating that GABAB modulation with ageing may be muscle group dependent. Furthermore, variations in SP duration and LICI modulation during and post fatigue in both groups suggest that these measures are likely mediated by divergent mechanisms.

The creation and viewing of war films was one of the elements in the process by which Britain attempted to come to terms with the horrors of the First World War. During the interwar period, war films took two main forms: those which reconstructed famous battles and melodramas set against a wartime backdrop. However, the film Blighty, directed by Adrian Brunel in 1927, took a slightly different approach, focusing not on military action but on those who stayed behind on the Home Front. As a director during the silent period, Brunel trod a stony path, operating largely on the fringes of the industry and never really getting a firm foothold in the developing studio structure. He remains well regarded for his independent productions yet also directed five features for Gainsborough at the end of the silent period. Of these film, his first, Blighty, is perhaps his most successful production within the studio system in terms of managing a compromise between his desire to maintain control while also fulfilling the studio's aims and requirement for box office success. Brunel's aversion to the war film as a genre meant that from the start of the project, he was engaged in a process of negotiation with the studio in order to preserve as far as possible what he regarded as a certain creative and moral imperative.
